How do I get started with Pathways in Care?

Getting started is easy. Just complete our contact form with your email and phone number, and we’ll be in touch promptly to discuss your situation and how we can best support you.

What areas do you cover?

We currently support families across the Perth Metro area. If you’re outside this region and need advice or assistance with aged care, please get in touch—we’ll do our best to help or connect you with the right support.

Aged Care Placements

How long does the Aged Care Placement process typically take?

Timeframes can vary depending on your situation. In some areas, waitlists can be up to a year. We work closely with you to find the best available options for your needs and advocate strongly to help secure a placement as quickly as possible.

What is the difference between the Basic and Complete Placement Services?

Basic Service – We guide you through what's required to enter Aged Care, give you the resources to manage the process yourself, and recommend suitable facilities to contact.

Complete Service– We take care of everything for you, from completing forms and contacting Aged Care homes to arranging tours and managing placement offers. Once you choose a home, we organise the admission and keep everyone updated along the way.

In-home Services & Home Care Packages

What is the difference between Community Home Support and Home Care Packages?

Community Home Support gives you basic help at home, such as cleaning, showering assistance, transport, or shopping. It’s quicker to set up because you don’t need a full Aged Care Assessment (ACAT). Support is usually limited to around 2–3 hours a week.

Home Care Packages provide more flexible, ongoing support. They do require an Aged Care Assessment and are available in four levels, each with a set amount of funding. Higher levels (3 and 4) offer more hours and services, helping people with greater care needs to stay at home. From 1 November 2025, the new Aged Care Act will make some changes to how these packages work.

Can you help me if I already have a Home Care Package?

Yes. If you’re not getting the most out of your package or the services don’t suit your needs, we can step in on your behalf. We’ll work with your provider to make sure you receive the care and support that’s right for you and your family.
